Return flight request: Departing from: (airport)/Arriving at: (airport) Date/Time: _____

Frequent Flyer# if applicable

Specific Flight Requests (e.g. airline, special meal)

TSA policy requires travelers to provide name (as it appears on government-issued ID the passenger plans to use when traveling), date of birth, and gender – see http://www.tsa.gov/stakeholders/secure-flight-program for more information.

Travelers should request the lowest fare tickets possible. If travelers plan to extend their stay on either end for personal reasons, an airfare quote for the business portion of travel only should be obtained from UC Travel at time of ticketing to demonstrate that the addition of personal travel is not increasing the cost of the airfare.  Please be sure to submit documentation of any airfare comparisons from UC Travel with your reimbursement paperwork.  If the cost of the airfare with personal travel is higher, the traveler will be required to pay the entire cost of the airfare out-of-pocket and seek reimbursement for the cost of business portion following completion of travel.
